How to fight denials of coverage

Fighting coverage denials is just a part of having health insurance.If it hasn't happened to you yet then it will inevitably happen sooner or later.And if you're like most Americans you will probably just roll over and accept the claim denial and pay the big foot.There are things you can do however and to help you fight coverage denials.Here are some steps to take when trying to figure out how to fight denials of coverage with your Insurance Company.

Step 1:Know what your rights are
If you are denied coverage for a treatment or drug you are responsible for collecting information on the case and what kind of coverage you have.Check to see what your rights are under your health insurance plan and under the state law.Talk with someone in the human resource department to get a copy of your policy so you can read it through carefully and understand exactly what your coverage is.Your policy will be able to help you understand how to challenge the provider's decision; if you don't understand then ask someone.

Step 2:Contact the Insurance provider
Make sure that before you make the phone call that you have all the papers you will need together and in front of you so when they ask for account numbers and such you will have them ready.You will have to lay out all the evidence to convince your insurance provider that you are right and they are wrong.It may just take one phone call to resolve the issue or it may take several calls and transfers and talking with person after person.Make a file of your phone calls and what you talked about; keep accurate records of everyone you have contact with.You're more likely to not get strung along if you have evidence recorded of the last person you talked with, what they told you, the date, etc.

Step 3:File a written appeal
Everything holds better in writing, so if the phone calls don't seem to be getting you anywhere you can file a written appeal with the health plan providers.You will need to request a copy of the entire claim which will include specific rationale for rejecting your claim.You can attach any supporting documents you have and anything else that will help our case.For example, if the claim says that the treatment was unnecessary, then attach your medical records showing that it was not.Blood work, test results, and any other explanations.One of the biggest mistakes people can make when filing a written appeal for denial of coverage is not supplying enough evidence.More is better than less so makes sure you are thorough in your process.

Step 4:Ask for a review of your case
If everything else has failed you can go right to the insurance provider and demand an independent review.Reports have shown that at least fifty percent of cases that are independently reviewed are won by the employee so your chances are fairly good at winning the case.

Being denied coverage of insurance is frustrating and the initial though is to just pay it or take it.More times than not you will have a legitimate case in fighting denials of coverage so buck up and take on the system.
